Parents are able to access and download a number of forms required to support your child's education. Please check to see if forms are available online to save you time, although if you cannot find what you need online or you still have any questions please feel free to come in and see one of our helpful Administration Staff.
Student Health Forms
It is vital that the school is well informed about students health needs and that any plans are in place to minimise risk to all students. We ask that parents promptly and regularly update medical forms. There are two forms Forms which need to be completed. The first is the STUDENT HEALTH CARE SUMMARY, which is complete for all students. The second form is the HEALTH CARE MANAGEMENT & EMERGENCY RESPONSE, which is complete for those students with identified conditions that staff need to be aware of and that may or may not have treatments/medications at school.
Students from Surrounding Areas
Bus services do run to Three Springs Primary School for those students outside of the town on farming properties. Parents need to apply directly to School Bus Services using the online application system. More information about who can apply and where to go to apply can be found on the information form below.
